The entrance to Childrens Hospital is on Sunset Blvd, pull into the main entrance and there is patient parking underground or your can choose to use valet services in the front.

The office for urology is located in the outpatient tower third floor, located behind the main hospital. Please obtain a visitors pass/sticker from the main entrance and continue behind the main entrance, pass the surgery center.

Where do I register?
Registration for an Office Visit is on your right as soon as you enter the Outpatient tower. Please look for the Urology Registration Sign.

What should I bring to the office visit?
You should have with you your insurance card and a contact address, telephone number, and fax number for your pediatrician or referring physician, and completed pre-registration form.
